V355/5 separates manufacturer, make and model/full specification into boxes 5, 6 and 7. It says the details are taken from the Certificate of Conformity, IVA or GB conversion certificate where applicable.

Use the make wording that best matches the official vehicle evidence.
Keep trim, engine variant and full specification details for box 7.
